Home care can mean you return home from the hospital sooner or, in some situations, avoid hospitalization altogether. Along with professional medical care of the highest standards, home care offers you support in maintaining normal activities when a health problem affects your quality of life.
The home care team of Community Health and Counseling Services will help you to recuperate from an operation or illness, assist you in managing a disability, or support you and your family in adjusting to a terminal illness.
Plans for your home health care are developed with you and your family by a CHCS staff member and administered under your physician's direction.
Health services are provided in the privacy and comfort of one's home. In many cases home health care can be a welcome alternative, emotionally and economically, to institutional care. Bringing skilled nursing and other health services intermittently to the patient at home may shorten a hospital stay or may substitute for full time residence in a nursing or convalescent home.
